Over 75s Named GP

From April 2014 all practices are required to provide all their patients aged 75 and over with a named GP who will have overall responsibility for the care and support that they provide them.
The named GP will work with other relevant health and care professionals to ensure that patients care packages meet individual needs.
All patients aged 75 or over will be informed by letter or e-mail or in some cases, this information may be passed to you with your next prescription.
Patients will be allocated their named GP before the end of June 2014 or within a month of registering with the practice. HOWEVER, you can still see the GP of your choice, whenever you need to see a doctor - you don’t have to see your named accountable GP if you prefer not to.
Please note, this does not mean that your named GP will be available to see you or speak to you at all times - the intention is that a named GP keeps a general eye on what is going on with your care, and makes sure things happen when needed. In many instances they will delegate someone else or some other organisation to sort things out. However, we have tried, as far as possible, to allocate a named GP who is already the GP best known to you.
